As theNurse Care Coordinator of our Assisted Living, you will get the opportunity to grow as a nurse.

Requirements:

RN preferred, but not required - can be an LPN
Experience with geriatric residents preferred
Long-term care experience beneficial, but not required
Excellent verbal and interpersonal communication skills

Hours of work:

Full-Time: 40 hours a week
Primary Office Hours are 8:30 AM-5:00 PM
Potential for on-call as needed

Job duties include but are not limited to:

Coordinate Medication Assistance Program
Complete nursing assessment for resident urgent needs
Participates in admission and annual assessment processes; maintains Service Plans
Guide care staff regarding resident needs and changes
Correspond with family and physicians regarding resident concerns or needs.
