Program Overview:

Located in downtown San Diego, CA, the Areta Crowell Centeris an adult outpatient program which provides a variety of intervention services to individuals in crisis. We serve individuals with serious mental illness and co-occurring substance abuse disorders.

Our services include initial psychosocial assessments, psychiatric and medication evaluations, ongoing medication support services, limited individual and group counseling, supported housing, care coordination, case management, and linking clients with additional community-based supports.

Typical Duties of Discharge Planner

  • Assess discharge needs of each person served and coordinates discharge planning activities to ensure continuing care supports are in place as needed
  • Collaborates with the treatment team to meet discharge goals
  • May assist with initial assessments and admission to the program as well as provide screenings, assessments, crisis intervention, and outreach activities to residents at the program
  • Represent the program/CRF within the community and counsel clients regarding supports and resources available

Required Skills and Qualifications

  • Masters degree in Psychology, Social Work,Counseling or othermental health related field
  • Minimum of two years of experience providing related services is ideal
  • Experience working with clients with persistent and serious mental illness in a high volume environment would be an advantage
  • Ability to maintain calm demeanor in crisis situations is essential
  • Proficiency in MS Office applications, specifically Word and Excel
