Hours: 40 hours per week. Clinic hours are 8:30 am - 5:00 pm, Monday - Friday

Program Overview:

Located in San Diego, CA, the Areta Crowell Wellness and Recovery Center is an adult outpatient program which provides a variety of intervention services to individuals in crisis. We serve individuals with serious mental illness and co-occurring substance abuse disorders. This program is located in the Mira Mesa area of San Diego County.

Our services include initial psychosocial assessments, psychiatric and medication evaluations, ongoing medication support services, limited individual and group counseling, supported housing, care coordination, case management, and linking clients with additional community-based supports.

Typical Duties of the Assessment and Discharge Specialist

  • Provides screening, clinical assessments, case management and outreach for outpatient program clients diagnosed with severe mental illness and may be dually diagnosed with a substance abuse disorder
  • Conducts collateral interviews, develops individual service plans and participates in interdisciplinary treatment planning
  • Provides crisis intervention as appropriate
  • Coordinates with the multidisciplinary team regarding discharge planning in progress
  • Collaborates with nursing staff to arrange continuing care medication maintenance
  • Assists clients with obtaining treatment benefits and other community-based supports to facilitate recovery and improvement

Qualifications/Skills Requirement

  • Master's degree in Psychology, Social Work or Counseling
  • Must be a registered ASW, APPC, MFA or Waivered Psychologist.
  • Minimum of 2 years of experience with working with individuals who have a severe and persistent mental illness, as well as co-occurring disorders required
  • Those with experience in a walk-in outpatient-based mental health program encouraged to apply
  • Proficiency in MS Office applications, specifically Word and Excel required
  • Must be able to work in a fast-paced and collaborative environment
  • Able to pass criminal/background clearance
