Mental Health Counselor / Aftercare Case Manager Mental Health Services for Short-Term Acute Residential Treatment (START) Program

Hours:40 hours per week; Sunday, Monday, Tuesday, 8:00 am to 4:00 pm, Wednesday and Thursday, 4:00 pm to Midnight

Program Overview:

Located in Oceanside, CA, the Turning Point Crisis Center provides intensive, community-based crisis residential services for adults throughout San Diego County. We deliver a unique alternative to psychiatric hospitalization to clients who are suffering a psychiatric crisis of such magnitude so as not to be manageable on an outpatient basis. Our services are provided in a home-like environment that specializes in the psychosocial rehabilitation of each individual.

Built around a bio-psychosocial approach, our multi-disciplinary team addresses each aspect of a clients unique situation, including mental health, co-occurring substance abuse, housing, medical/medication needs, and establishing connections to social service supports within the community to facilitate successful transition beyond the START program.

Typical Duties of the Mental Health Counselor / Aftercare Case Manager

Meets the bio-psychosocial needs of clients by providing indicated behavioral health services, case management services and/or crisis intervention, and documents services appropriately. Works with individuals to assist them in accessing appropriate support services and entitlements, medical care, community resources, and family support systems in an effort to facilitate recovery and rehabilitation.

Duties include, but not limited to:

  • Conduct initial psychological assessments for clients, utilizing a psychosocial rehabilitation focus and dynamic team-based approach
  • Conduct individual and group counseling/psychotherapy sessions
  • Develop and update individual treatment service plans for clients
  • Create discharge plans and provide linkage to other support services within the community
  • Participate in interdisciplinary treatment planning
  • Assist clients in a smooth transition to outpatient medical and social services
  • Assist higher risk clients with solidifying strong discharge plans
  • Stay in close contact with clients (post discharge) to ensure they are successfully connecting with aftercare resources
  • Works closely with various providers in the region or system to facilitate access to services. Is well versed in community resources, providers, and partners available in the region
  • Develops and maintains professional working relationships with other programs and agencies
  • Conducts crisis intervention and care coordination including connecting clients with area resources
  • Consults and coordinates with family and/or support system regarding client treatment and discharge planning

Required Skills and Qualifications

  • Master's degree in a mental health-related field such as Social Work, Psychology, Counseling; Bachelor's degree in a mental health related field may be considered with successful completion of a CRF START internship or four years of related experience
  • Minimum of2years of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred
  • First Aid/CPR certification is required and must be kept current during employment
  • Proficiency in MS Office applications, specifically Word and Excel
  • Ability to thrive in a fast-paced environment is essential
  • A valid California drivers license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business
  • Ability to pass criminal/background clearance
