Annual Wellness/Advance Care Plan Coordinator (Full Time)

SouthCoast Health Is Looking For A Full-time Annual Wellness/Advance Care Plan Coordinator

Responsible for providing and conducting Advance Care Plans and annual wellness visits to our patients under the general policies and procedures established by SouthCoast Health. Provides care as allowed within the scope of the training or licensure.

Qualifications:

Ability to exercise professionalism in dealing with all levels of personnel, patients and patients' families.
Ability to communicate clearly.
Ability to work effectively as team member with physicians and other staff.
Knowledge and demonstrated proficiency in performing clinical skills.
Knowledge of medical charge reporting/records.
Knowledge of appropriate assessment and assistance techniques.
Knowledge of health information management by appropriately charting patient data.

Responsibilities:

Conducts Advance Care Plans (ACP) according to guidelines set forth by Medicare and ensure they are within insurance plans timeline.
Document Advance Care Plans (ACP) in the patient's electronic medical record.
Provide patients with the SCH ACP document packet during the visit.
Develops patient preventive reports in line with the Annual Wellness Visits guidelines.
Maintains confidentiality in matters regarding the clinic, its patients and employees.
Perform clinical and non-clinical duties as assigned by Supervisor.

Education/ Experience:

Completion of an education program that meets the requirements of a qualified health professional per CMS regulations (Example: Nurse, Extender, Pharmacist, Social Worker, Dietician). Minimum of one year clinical experience, preferably in a hospital or office practice setting.

Requirements:

Valid and current Georgia License Master Social Worker (LMSW) or Georgia LPN or RN licensure. Current BLS/CPR Certification.
