Associate Medical Director/Physician - TRUST Clinic - Downtown Oakland (28 Hours Per Week) (Part Time)

LifeLong Medical Care has an exciting opportunity for a Physician to join our multi-disciplinary team providing primary care services at TRUST, Alameda County's collaborative project with LifeLong to provide a unique model of care for homeless or at-risk of homeless adults, many with mental or physical health disabilities. The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, and ideally experienced working with the homeless, underserved, and/or those with disabilities.

About the Clinic

The TRUST Clinic is a collaboration with Alameda County Health Care for the Homeless Program and Behavioral Health Care Services. TRUST will open this summer in downtown Oakland providing a primary medical home for this at-risk population. Case management and behavioral health services will be fully integrated into the model which will be staffed and structured to address a range of medical and psychosocial factors impacting health.

Key Duties and Responsibilities

We are looking for a physician with administrative experience who is committed to high-quality primary care and has a passion for developing and participating in a highly collaborative team model of care for a complex population. Duties include:

Provide leadership at the TRUST Clinic assuring clinical quality.
Participate in clinical leadership with other Associate Medical Directors and the CMO.
Supervise physicians and advanced practice providers
Collaborate with partner agencies to develop an integrated system of care for the target population
Key role in provider recruitment and retention
Assist the Center Director with budget development, financial performance of the clinic, and clinician staffing.
Assist with development and implementation of organization and site quality improvement initiatives.
Participate actively in collaborative care, team building and training
Provide direct patient care (position is a blend of clinical and administrative duties - approximately 70% clinical).

Qualifications and Experience

Individual must be a physician possessing current CA Medical Licensure and at least one year related experience with multi-cultural, high-risk patients in outpatient primary care.
Prior clinical management experience preferred.
Familiarity with electronic prescribing and electronic health records desirable.
Ability to work effectively and collaboratively in a collaborative, fast-paced primary health care setting and prioritize competing job demands.
Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds, particularly with the homeless population and individuals with mental health and/or substance use concerns.
