Care Coordinator - Care Coordination Unit (Temporary)

General Purpose:.
Provide coordination of Medicaid and non-Medicaid services for consumers with a developmental disability/mental retardation. |
Essential Functions:
Maintain contact with consumers, families, to assure quality service. Assist developmentally disabled consumers to develop and implement an Individualized Service plan in accordance with Medicaid guidelines. Coordinate and refer consumers to appropriate services to help them meet goals as specified in ISP plan. Maintain Case file in audit ready condition and conduct monthly meetings with consumers. Coordinate all services as stated in ISP Responsibility for developing ISP subject to review of Program Manager Manage caseload of 25 consumers (in accordance with OPWDD regs.) Responsibility for maintaining Medicaid records according to billing requirements and all audit standards. Generally provide a variety of professional supports. Maintain Medicaid Eligibility in order to bill for service Provide Authorizations for various waiver services. Write clear and concise case notes in a timely manner. Work in a cooperative manner with other Goodwill and non-GW programs such as supported employment, and other Waiver and non-Waiver services. Assists consumer in achieving the highest level of independent functioning in the community. |
Qualifications/Basic Job Requirements:
Bachelor's Degree required in psychology or related field (social work, sociology, mental health, etc.) Two years experience working with OPWDD . Developmentally disabled population

Scope of Responsibility & Positions Supervised:.
Supervised by Program manager for Medicaid Service Coordination. |
Special Working Conditions:
Job primarily consists of field visits to work place, family, other service agencies, and locations where services can be obtained (libraries, schools, parks, etc. to reach the goals of the ISP. |
