Join the dynamic Private Duty Team at Horizon Home Care and Hospice, located in Milwaukee, WI as a RN Case Manager.
This unique role blends your love for connecting with patients while providing comprehensive care oversight toinclude initial referral, admission, care development and coordination, and ongoing case management and your desire to grow your leadership skills by assisting in the mentoring and supervision of private duty home health aides and personal care workers; all while enjoying the work/life balance that comes with a full-time, first shift position. This multi-layered position works collaboratively with other team members including the Home Health Aide Supervisor, Home Health Aide Coordinator, Team Liaison and Director to assure the highest level of care is provided.
This ideal first shift positions works Monday-Friday, 40 hours per week; with 1 weekend a month on call with pay. Qualifications include a degree from an accredited School of Nursing (BSN preferred), current WI RN license, and 3 - 5years ofrelated home care and/or supervisory experience.
