As a MyChoice Family Care contracted CMU, Horizon's Family Care staff emphasize preventative and holistic care. The half office, half field Registered Nurse/Case Manager position provides invaluable support to Milwaukee's growing geriatric, DD, and behavioral health community. Help Milwaukee residents maintain their independence in the home as long as possible.Meanwhile, help yourself maintain an optimal work/life balance.
Thisfull-time position works desirable 1st shift hours Monday through Friday. NO WEEKENDS!!

Requirements includea minimum ADN degree (BSN preferred), RN license to practice in the State of Wisconsin, andat least one yearof direct care experience with geriatric, developmentally disabled, or behavioral health population. This position is based out of our Park Place (Northwest Milwaukee) corporate headquarters but alsotravels on patient visits throughout Milwaukee County. A valid Wisconsin driver's license and auto insurance are mandatory.
