Our Hospice team is looking for a compassionate, personable Medical Social Workerto provide initial psychosocial evaluations and ongoing psychosocial counseling to our patients and their families.

This rewarding Pool, On-Call opportunity allows you to collaborate with an inter-disciplinary team, work with the patients in the comfort of their own homes, and really make a difference; all while keeping your calendar open during the week for school, work or other obligations.

Qualifications include a required Master's in Social Work and current State of WisconsinSocial Worker licensure. Must have a working knowledge of the principles and practices of social work, awareness of community resources, ability to relate to people of diverse cultural backgrounds, utilization of various treatment modalities, and the ability to work within an integrated team concept. Must have at least two (2) years experience in a clinical setting, preferably hospice-related. Current WI drivers license, reliable transportation, and proof of current automobile insurance also required.

Be advised that you must be available for 3 consecutive weeks and several additional days of Mon-Fri 1st shift Orientation.
