Our Intake Department, located at our Milwaukee Park Place corporate headquarters, has an immediate need for a Registered Nurse - Care Coordinator to work during the day Monday Friday with limited Holiday rotation. In this position you will work with patients, families and physicians to arrange for homecare services keeping the person where they heal best, at home! r. Intake is the hub of our Agency, and we need a nurse with the utmost organization and communication skills to help these most vulnerable in our community. Be prepared to be challenged and grow in your practice every day as you work with an amazing, collaborative, fun, and professional team.
Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, at least two years of experience, and computer/typing proficiency. Home care experience is preferred but not necessary. A commitment to unparalleled customer service and fitting in with out collaborative culture are essential. Must be adept at communicating with various departments, and other healthcare professionals. A willingness to learn how to use multiple electronic health records is essential.
