Our Family Care Team is looking for its next leader to continue to build upon our tradition of excellence, quality, dedication and compassion to our members.

As the leader of our Family Care CMU, the Program Manager will be responsible for day-to-day coordination and supervision of all aspects of our contracted program. This unique and dynamic role will ensure Horizons compliance with our MyChoice Family Care contract and care management operations; ensuring appropriate staffing to support member enrollments, which includes direct supervision to supervisory team and program staff as needed; confirming documentation standards, quality and regulatory compliance are met and providing support, guidance and direction regarding sensitive and complex member situations.

Qualifications include: MSW and social work licensure in the State of Wisconsin, plus 2 years of supervisory experience working with specific populations (e.g. DD, ID, PD, Frail Elder) in social service care management or related field. In addition, we are seeking a leader that has awareness of community resources, ability to relate to people of diverse cultural backgrounds, understands the dynamics of human behavior/social issues, has practiced the use of various treatment modalities and has had prior experience working in an integrated team. Must also possess a valid WI drivers license, reliable and insured automobile.
