We are currently seeking a full-time Nurse Practitioner to provide medical services including, but not limited to, assessment, treatment, and health teaching in the form of patient home visits. Some responsibilities include:

Consult with the patients attending physicians, as needed, regarding management of wound care, illnesses and chronic conditions, in addition, meet the general medical needs of the patients. Conduct face to face encounters for assigned patients. Act as a resource to staff who identify patient issues relating to wound care and other specific clinical areas by recommending a plan of care and implementing when appropriate with the case manager and/or interdisciplinary team.

This position is based at our Milwaukee headquarters but also travels throughout our service areas; including Kenosha, Milwaukee, Waukesha, Ozaukee, Racine, Washington Counties as needed.

Qualifications include a current State of Wisconsin APNP license, plus at least 2 years of home care and/or similar clinical experience.
