Our inter-disciplinary Hospice team provides the full spectrum of care for our Hospice patients and offers multiple levels of support for their loved ones. From our free Grief Resource Center to our award-winning Virtual Hospice at Froedtert, to our many hospice enhancements (massage therapy, pet therapy, music therapy, spiritual guidance, etc.), we tirelessly strive to create the most positive hospice experience possible.
During that critical period near the end of life, a hospice nurse is the person patients turn to for care, andfamilies turn to for comfort and education. Little by little, your efforts can have an amazing impact on the lives of these people. It is a meaningful responsibility and, for many, a calling.
Horizon is expanding our service area and therefore we have immediate opportunitiesavailable within Kenosha County. Our RN Patient Care Coordinators (Case Manager) enjoy a desirable Monday-Friday, 1st shift schedule with NO On-call and NO Weekends!
Requires a degree from an accredited school of nursing (BSN strongly preferred), a valid WI RN license and 1-2 years of relevant experience.CHP certification strongly preferred.Because this position travels throughout SE Wisconsin you must have a valid WI drivers license and auto insurance.

Qualifications include a degree from an Accredited School of Nursing, WI Registered Nurse license, andat leastone year of previous nursing experience; home care/hospice preferred. HPNA certification as a CHPN preferred.
