RN - Intake Care Coordinator  (Full Time 1st shift)

Only an upcoming retirement makes this amazing opportunity available within our Intake Department, the hub of our growing Agency. We are currently seeking a full-time RN Care Coordinator at our partner hospital, Columbia St. Marys Hospital Milwaukee located on Lake Drive in Milwaukee, WI to assess, develop care plans and work with hospital case management to coordinate the admission of patients to Horizons home care services.

We need a nurse with organizational and interpersonal communication skills to join our fun, professional and dynamic Intake team. Enjoy a salaried position working desirable 1st shift hours Monday-Friday with NO ON-CALL AND NO WEEKENDS that will challenge you and allow you to really make a difference in our patients lives every day.

Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, at least 3-5 years of related experience, and computer/typing proficiency. Home Care experience required.
