As a contracted CMU servicing a Title 19 managed insurance program through My Choice Family Care, Horizon's Family Care staff emphasize preventative and holistic care. This RN position provides invaluable support to Milwaukee's growing geriatric, DD, and behavioral health community. Help Milwaukee residents maintain their independence in the home as long as possible.Meanwhile, help yourself maintain an optimal Life/Work balance with a desirable 1st shift schedule.

Requirements includea minimum ADN degree (BSN preferred), RN license to practice in the State of Wisconsin, andat least one yearof direct care experience with geriatric, developmentally disabled, or behavioral health population. This position is based out of our Park Place (Northwest Milwaukee) corporate headquarters but alsotravels on patient visits throughout Milwaukee County. A valid Wisconsin driver's license and auto insurance are mandatory.
