Position Summary:The Community Care Coordinator is responsible for providing strengths based case management, advocating to ensure the basic needs of the child/youth are met, guiding the multi-disciplinary treatment team in treatment & discharge planning as well as ensuring high quality communication & documentation with internal and external parties involved.

Tasks are performed under the direction of the Community Care Director on the assigned unit.

Essential Functions of the Position:

Position Requirements:

Experience:At least two years experience in providing direct care in a residential setting or 2 years of case management experience is required. Knowledge of all aspects of residential treatment and strengths based case management is preferred.

Education:A Bachelors degree in the social work, psychology or a related social/human services field is required. Two years experience in working with juvenile offenders and children in need of care is preferred, along with knowledge of residential programming.

Required Skills:This employee must have the ability to work with youth who have emotional and behavioral challenges. The employee must be able to demonstrate effective and effective communication with coworkers from a variety of discipline. The employee must have excellent interpersonal, oral, record keeping, and written communication skills. The employee must have the ability to express of exchange ideas by means of written and verbal communication with internal and external agencies that relate to the PRTF facility. The employee must have the ability to establish a rapport with people easily that involves establishing a climate of trust, confidence, and mutual respect. The employee must have the ability to work independently as well as collaborate effectively with others. The employee must have the ability to view a client with his or her strengths in mind and with an awareness of diverse populations.

Cultural Competency: All KidsTLC employees are required to accept the ethnic differences between people and be willing to work with clients of different ethnic groups. Each employee is also required through education, training, practice and self reflection, to gain knowledge of cultural groups, the role of language, speech patterns, and communication styles, and of the impact of social service policies on clients. Every employee must recognize the ways that professional values may conflict with or accommodate the needs of minority clients, must have the ability to openly discuss racial differences and respond to culturally based cues, and have the ability to recognize and combat racism, racial stereotypes, and myths in individuals and institutions.

Physical Requirements:Some physical requirements in working with agitated juveniles and their parents/guardians are applicable to this position. Physical requirements include the ability to physically restrain agitated youth utilizing Emergency Safety Physical Intervention (ESPI) techniques. While restraining, employee must have the ability to safely get down on knees and remain in position for up to 10 minutes, must have extreme flexibility to hold youth and switch positions during restraint, and the ability to lift, push and pull up to 40 pounds. Applicant/Employee must be at least 21 years of age with valid drivers license and satisfactory driving record.

Problem Solving:Problem-solving skills are a necessary component of this position. Problem areas include but are not limited to the following: personnel and shift performance; treatment planning and scheduling issues; transportation issues; program implementation and training issues; community resources; family and juvenile issues.

Decision Making:Decision-making skills are a necessary component of this position. Decisions may be required in the following areas: treatment planning and scheduling issues; transportation issues; program implementation and training issues; community resources; family and client issues.

Personal Relations:This position will have frequent contact with youth and parents/guardians, case managers and clinical personnel in outside agencies, probation staff, child welfare providers, juvenile justice providers, KidsTLC support staff, PRTF staff, KidsTLC Operations Management staff, treatment team members, Council Of Accreditation (COA) and its committees. It is imperative that the incumbent to this position engages in positive and constructive interaction with all individuals in which she/he may interact.

Working Conditions:The hours of this position will include a 40-hour work-week. The incumbent to this position must be flexible with the work schedule and expect schedule changes as required by agency needs. This employee must professionally interact with various KidsTLC staff, providers, clients, and guardians as necessary.
