Bilingual Case Manager- PIPBHC  (Full Time)
Location: Woonsocket, RI

Position Responsibilities:

Provide care coordination between providers in various settings to improve participation and engagement

Provide individual and familytreatment for children,adultsand family memberswhomeet PIPBHC criteria

Services take place in a variety ofsettings and are flexible in nature

Case Managers function as part of a team and are required to collaborate with other team members as well as other agenciesincluding FQHCsand individuals involved withidentified families

Collaboration with regional FQHC including utilization of the integrated treatment plan

Model and support health and wellness activities and healthy coping skills forPIPBHCfamilies

Provide case management support and advocacy to participants and their families to eliminate any barriers to participants recovery

Assess clients on an ongoing basis, provide support services and crisis intervention as needed

Education and Licensing Requirements: Must have a Bachelors degree in Social Work or related field. Valid drivers license, auto insurance andreliabletransportationis required.
