RN Case Manager (Graceworks at Home) (PRN)
Location: Dayton
PRN: Responsible for assessing, planning, implementing, and evaluating the nursing care for a caseload of clients from admission to discharge. Coordinates and supervises the nursing care delivered by the home health aides working under the supervision of the Director of Graceworks at Home, LLC. Assumes case management responsibilities as assigned. Must be a Registered Nurse from an accredited school of nursing with a current Ohio license; CPR certification and possession of a valid driver’s license with a safe driving record required. Two years of nursing experience, one of which involved home health, preferred.
