Physical Therapist (Health at Home) (PRN)
Location: Dayton
PRN: Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all client programs for clients of Health at Home, LLC, an affiliate of Graceworks at Home. Administers physical therapy in compliance with Federal, State, local, and Health at Home standards, policies, and guidelines ensuring quality care and service. Qualified candidates will have a Bachelor’s degree from an accredited college in Physical Therapy (Master’s degree preferred), current Ohio Physical Therapy License and CPR certification. Valid drivers’ license and safe driving record required. At least two years physical therapy experience in long-term health care setting preferred.
