RESPONSIBILITIES
The Community Health Worker establishes trusting relationships with individuals and their families to uncover barriers that are keeping him/her from managing a chronic illness and living a healthy life; provides assistance in addressing the social determinants of health, developing a care plan, implementing the care plan, and providing referral and linkage to supports for the individual; care plans may include assisting with transportation, addressing housing and/or food insecurities, connections to workforce resources, connection to health insurance and/or medication resources, providing health literacy and building trust.

REQUIREMENTS
Minimum Requirements
M1:Associates degree
M2:Valid drivers license and safe driving record

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.

ANNUAL SALARY RANGE
$31,856 - $55,748
