Intensive Care Coordinator (4275) (Full Time)

RESPONSIBILITIES
Provides intensive case management services to children and adolescents at risk of entering or currently placed in a residential facility; conducts comprehensive strength-based assessments; identifies appropriate services and implements plans of care; coordinates service delivery with providers and community resources; evaluates provider quality; provides on-site visits; and completes required documentation.

REQUIREMENTS
Minimum Requirements
M1: Bachelors degree in psychology, social work, counseling, or other VA DMAS approved Human Services degree
M2: At least one year of experience within the past seven years providing mental health services to children and adolescents
M3: Valid drivers license and safe driving record

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.

ANNUAL SALARY RANGE
$42,865 - $75,014
