Summary

The Director of Quality will supervise and coordinate HealthCore Clinic??s (HCC) quality improvement and information management processes through performance measurement/assessment/improvement, information management activities, accreditation, consultation, training, and other quality issues. 

 

Job Description

This position requires an individual who is self-directed, a team player, organized, and has an ability to critically think and problem-solve. The Director of Quality will lead HCC??s quality improvement initiatives including establishing policies, procedures, workflows, and standardized processes aimed at improving quality of care and the patient experience. In addition, the Director of Quality will support informatics/reporting, Electronic Health Record (EHR) improvement, data management, grant writing, and research activities. 

Job Responsibilities.  

·         Lead and document HCC??s quality improvement initiatives using quality improvement methodologies and/or evidence-based practices.

·         Measure strategies to achieve accreditation to be a patient centered medical home. 

·         Coordinate accreditation processes and survey activities throughout the organization.  

·         Work with quality committees, departments, and programs to meet accreditation standards and to comply with regulations.

·         Establish, implement, and maintain effective formal communications systems and processes relating to performance and accreditation activities.  

·         Provide assistance in the development of data collection systems that monitor functions and processes.  

·         Assist with system development by identifying data sources and retrieval methodologies. 

·         Assist HCC with procedures affecting documentation such as workflows, record reviews, etc.

·         Provide consultation to departments facilitating and/or participating in the design of new processes, programs, reports, and data collection as necessary. 

·         Facilitate annual provider site visits with accreditation bodies as well as funders while encouraging quality and performance improvement activities.

·         Monthly review and clarification of HCC??s quality report; program strengths; current performance and quality improvement initiatives; discussions related to the facility??s model of care and services provided.

·         Support data management activities including development of documentation strategies, report writing, and data analysis

·         Develop and provide training to employees in areas relating to performance management, data collection, analysis, and other areas as needed. 

·         Support development, implementation, and training of new/improved EHR features, workflows, and processes.

·         Support grant writing and research activities.

·         Maintains partnerships with universities and local community organizations.

·         Other duties as assigned 

Qualifications and Requirements

·         Bachelor??s Degree in non-profit management, public administration, quality management, business, social work, health care administration, nursing, or related field. Prefer Master??s Degree. Successful experience in performance management or continuous quality improvement. Strongly prefer accreditation experience.

  • Experience with eClinicalWorks a plus
  • Effective supervisory and leadership skills and abilities.
  • Well-versed in process improvement, data collection and analysis techniques.
  • Planning and organization skills.
  • Strong word-processing, spreadsheet, and database skills.
  • Knowledge of agency services and processes.
  • Ability to identify needs and solutions.
