Job Purpose: The OneCare Kansas is responsible for assessing patient’s needs and working with counselors and other care providers to design treatment plans, set up referrals and choose the most suitable resources for a patient’s specific mental health needs. The OneCare Kansas Coordinator will also be responsible for follow up and documenting progress on each patient. This position will also develop and provide behavioral health education to individuals, groups and the surrounding communities and conducts limited community assessments.
Patient-Centered Medical Home Essential Functions:
1. Patient Population Management:
a. Prepares patients reports in a timely objective manner, documenting all activity and communication by providing a comprehensive written report as required by company guidelines.
b. Reviews reports daily (Manage Clinic Schedule to help coordinate care for the patients coming in for various services).
c. Contacts all unreached patients to confirm that the patient will be present at his/her appointment.
d. Assists patients in navigating the health care system. Coordinates specialty care, follows-up on test results and other care coordination needs.
e. Evaluates ongoing patient progress and documents in EMR.
f. Maintains and coordinates data with our Quality Department to support the various Behavioral Health Programs (BHC/Integrated Care, MAT, and Psychiatry).
g. Works closely with the entire Behavioral Health team, providers (PCP’s, LMSW’s/LSCSW’s, BHC’s), medical assistants and other departmental care coordinators to provide the best care for the patient.
h. Reviews data reports and reporting to the various department meetings on various measures
i. Other duties as assigned.

- Bachelor’s degree in social work, psychology or counseling required.
- Two (2) to Three (3) years of previous experience as a care coordinator required; an equivalent combination of education and experience may be considered.
- LBSW or LMSW with LAC required, LCAC preferred
- Previous EMR experience preferred.
- Ability to communicate effectively with vulnerable populations including those made vulnerable by their financial status, personality characteristics, place of residence, health status and/or age.
- Ability to multi-task and organize work effectively while working independently and/or as a cooperative team member.
- Remain flexible to with stand unpredictable changes to workflow and schedules.
- Ability to maintain absolute confidentiality in regards to HCC matters.
