Minimum Education/ Experience:  A High School diploma or national equivalent from an appropriately accredited institution (GED) is required.

Active NC Driver’s License required.

 Brief Description of Work Performed:

TLC is seeking outgoing, compassionate and dedicated folks to provide services to individuals with intellectual developmental disabilities and behavioral health challenges in home-based and community-based settings.  Home and Community-Based Services are challenging and require patience and commitment.   As a provider, you will work with individuals who may require assistance with ambulation, mobility, refreshing, personal hygiene, meal preparation, medication administration, and seizure management protocols.    You may also work with individuals with challenging behaviors.

1)     HCBS Community Living Support (CLS) Provider

As an HCBS Community Living Support Provider, you will provide services that assist individuals with achieving their personal goals as outlined on their treatment plan.   These services are provided in community settings and in the home.  You will empower individuals to become more independent, learn to connect with others in the community, learn skills such as shopping, banking, cooking, personal care, hygiene, and activities of daily living. The intended outcome of this service is to increase or maintain the person’s life skills and maximize his or her self-sufficiency and self-determination.   Applicants must be at least 18 years of age and provide your own transportation to and from the location of the individuals as well as transport the individuals within the community.

 2)     HCBS Community Networking Provider

As an HCBS Community Networking Provider, you will help individuals find access to volunteer work, classes, and activities that involve them in the community. Community Networking provides individualized day activities that support the individuals’ definition of a meaningful day in integrated community settings.  The goal is to empower individuals to gain skills to increase community connections and increase their natural support network.   Applicants must be at least 18 years of age and provide your own transportation to and from the location of the individuals as well as transport the individuals within the community.

 3)     HCBS Respite Provider

As an HCBS Respite Provider you will provide services to individuals which may include personal care, activities of daily living, companionship, and participation in leisure activities.   This service enables the primary caregiver(s) to participate in planned or emergency events.   The service is for the caregiver(s) and enables them time to grocery shop, go to the doctor, have a date night, spend time with other children, go on an overnight or staycation.   Respite may include in and out of home services and may be inclusive of overnights, weekends, or emergency care.   Applicants must be at least 18 years of age and ability to provide your own transportation to and from the location of the individuals as well as transport the individuals within the community.

 Hours/Days of Work:

Schedule determined by the HCBS Manager according to the needs of the individuals served; the periodic position which may grow to part-time or full-time; will work in the individual’s home and community; must work a minimum of 16 hours per month.

 Non-negotiable Pay Rates: $250 Joining Bonus to be paid on the 1st payroll

 Community Living Support and Community Networking Care Provider Pay Rate:  Hourly non-exempt pay rate of $13.00 per hour after training is completed; $10.00 per hour during new hire orientation, training and for non-billable work.

 Respite Care Provider Pay Rate:  Hourly non-exempt pay rate of $10.00 per hour after training is completed; $10.00 per hour during new hire orientation, training and non-billable work.

Hourly, non-exempt position; Travel is not reimbursed; Service area may include the following counties:  Wake, Durham, Johnston, Orange, Person, and Chatham.
