Clinical Care Coordinator  (Full Time)
Location: Lutherville MD

SUBJECT:                    HOME CARE CLINICAL COORDINATOR JOB DESCRIPTION

POLICY:                       The Home Care Clinical Coordinator is responsible for the maintenance of all doctors' orders and overall care coordination for all skilled, case-managed patients.  Service excellence should be a direct reflection of efficient care coordination and effective communication between all involved parties.

QUALIFICATIONS:

                                    1.   Meets the educational and experiential requirements of the state in which ACCESS is certified. H. S. Diploma required.

2.   Has a demonstrated ability to direct and supervise professional and ancillary personnel and deal tactfully with the client and community.

3.   Has demonstrated the mature judgment, intelligence and emotional stability required to solve problems encountered in rendering home care to patients and their families.

4.   Must possess an outstanding ability to relate to others; prior experience in a people/consumer directed profession preferred.

5.   Able to meet the physical requirements of the position.

6.   Preferred: Prior coordination or office management experience.

7.   Preferred: Currently licensed as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).

REPORTING

RELATIONSHIPS         The Home Care Clinical Coordinator reports directly to the Business Operations Manager or designee.

 

RESPONSIBILITIES:

1.   To Provide Coordination to all Skilled, Case-Managed Home Care Services.

a.   To assume responsibility of the coordination of all skilled, case managed patient services.

i.    Assures that each patient is serviced appropriately through the assignment of caregivers whose skills and abilities meet the patient's individual needs.

ii.    Maintains clear operational procedures to obtain prompt medical care for patients in case of acute illness or other emergency.

iii.   Assists with coordination and delivery of information to appropriate personnel involved in patient care - specifically changes in patient status and/or needs.

iv.   Provides caregiver(s) with patient specific materials and maintains complete and accurate case management records.

v.   Contributes to the evaluation of all clients for purposes of continued need and eligibility for home health services.

vi.   Follows admission through discharge planning process to meet patient/family needs.

b.   To ensure that all patient services are managed according to ACCESS policies and procedures in a time and cost-effective manner.

2.   To Coordinate Services Provided By ACCESS.

a.     Documents all communication and schedule changes in system of record in a timely manner.

b.     Schedules appropriate personnel on case and staffing assignments according to patient, physician and family needs.

c.     Receives and forwards all complaints made by patient/families regarding services, personnel or policies to their manager, and works to resolve the complaint in the patient's best interests.

d.     Receives and documents all complaints/grievances made by caregivers regarding working conditions, personnel practices or policies and works to resolve the complaint in the employee's best interests.

e.     Document all incidents reported involving patients and/or employees and completes follow-up as appropriate.

f.      Assists in performance audits of case records according to ACCESS Quality Improvement policies.

3.   To Participate In All Activities Relevant To The Professional Services Provided, Including The Development Of Qualifications And Assignment Of Personnel.

a.     Manages all doctor's orders to ensure current orders are on file at all times which includes, but not limited to: updating all skilled, case-managed plans of care (485) every 60 days and all interim orders in between each plan of care (485) certification.

b.     Updates all MAR's and TAR's each month and mails updated record keeping documents to each skilled, case-managed patient before the first of each month.

c.     Tracks the receipt of all MAR's and TAR's for each skilled, case-managed patient for the prior month.

d.     Reviews MAR's, TAR's, and progress notes at least monthly - provides re-education when necessary. All communication and remediation to be documented in a timely manner in the system of record.

e.     Communicates and implements ACCESS administrative and nursing operational practices to all staff, patients, and families. Insures adherence to Patient's Bill of Rights.

f.      Develops techniques and methods for resolution of home care scheduling and assignment problems.

g.     Handles intake of referrals from doctors, hospitals, clinics and other referring agencies.

h.     Ensures caregiver compliance is maintained at all times.

i.      Contacts each patient/family at least monthly to determine satisfaction with services and resolution of problems. Also determines from the home care worker assigned if all services are being successfully delivered.

j.      Strives to maintain continuity of care by assigning the same home care providers to the client.

k.     Facilitates and documents a patient specific orientation to all newly assigned caregivers.

l.      Insures that only personnel who have successfully completed the screening process, and whose employment file is complete are scheduled for assignments.

4.     Maintains confidentiality of all patient and caregiver records.

5.     Participates in Performance Improvement Projects.

6.     Attends weekly office CCC meetings.

7.     Other Duties as needed and listed on the Responsibility Chart.
