THE PROGRAM

Vista Hill SmartCare is a unique program the offers comprehensive care, support and education to patients of rural medical clinics and its community members! SmartCare serves the communities of Ramona, Julian, Pauma Valley, Campo and Alpine. In this program our employees work autonomously to partner with local health care providers, identify community health and wellness needs, and find out-of-the-box solutions for an underserved population. THE POSITION

The Behavioral Health Consultant is a mental health clinician who works under the supervision of the Licensed Behavioral Health Supervisor/Program Manager, and provides behavioral health care of patients in the health clinic setting. He/she coordinates with the Primary Health Care team to ensure a seamless and integrated experience of care. The Behavioral Health Care Consultant is responsible for the clinical care of the patients, providing brief, strategic behavioral health interventions in the clinic, community and patients’ homes. You will have the opportunity to create, and participate in health activities, interventions, screenings and assessment. An example of tasks include:

· Receives “warm hand-offs” from Health Care Providers at health care clinic, as first contacts/ patients’ first exposure to SmartCare Integrated Behavioral Health.

· Provides screening and triages in the SBIR model within the context of a local health care clinic. Ensures that all patients feel welcome and that all issues, including co-occurring disorders, can be addressed through the clinical team and by supported referrals.

· Ensures that clinic meets Outcome goals: screening of all patients, Satisfaction Surveys of all patients receiving an intervention.

· Partner with the Behavioral Health Educators and the Health Care Providers, and appropriately advocates for the needs of the patient and partner with other agency personnel in the delivery of services.

· Assists Primary Health Care staff in understanding the range of alternative healing modalities that may be utilized, especially by Latino and Native American patients, such as the use of complimentary medicine and wellness activities.

· Provides consultation and professional development to primary care staff to promote integrated patient care and assessment.

· In collaboration with Behavioral Health Educators and the patient, develops a brief intervention plan, to include a transition/ discharge plan.

· Provides follow-up services (clinic based, community based or home based, per patients’ needs and comfort).

· Links patients in need of specialty mental health services to appropriate service providers.

· Maintains documentation (screenings, progress notes, wellness activity notes, Satisfaction Surveys, Discharge Notes) in the Care Manager system that complies with Vista Hill and County documentation standards and within required timeframes (within 2 days of service).

· Provides CSS treatment services to eligible patients, to accompany primary care practitioners’ treatment of uncomplicated SMI and SED patients when appropriate, preventing referral to separate mental health resources whenever possible.

· Develops and implements curricula for activities and classes such as stress management, healthy relationships, teen violence, drug and alcohol use, good nutrition and healthy recreational activities.

· Responds quickly and appropriately to crisis situations.

REQUIREMENTS

· Master's degree required.

· Licensed or Registered as an LCSW, LMFT, LPCC with California Board of Behavioral Science.

· Experience with health and wellness issues for both adults and children.

· Bilingual; English/Spanish preferred.

· Knowledge of East County rural communities preferred.

  • Ability to pass a drug, TB and background check, and COVID vaccine requirements.

PAY

Starting at $31 for Registered

Starting at $35 for Licensed

These are starting rates and may be adjusted up based on education and experience.

Current Vista Hill employees in these job titles for 2023 are paid:

$31 - $38.00 for Licensed

HOURS
Flexible - open to full or part-time.

BENEFITS

Employees working over 30 hours a week receive employer-paid benefits, including medical, dental, vision, paid time off (vacation and sick leave), paid holidays, employer-paid retirement contribution and clinical supervision (if applicable). Some programs qualify for student loan forgiveness programs.

Employees who work less than 30 hours a week do not qualify for healthcare benefits.
