Back to Category List Job Description Community Health Nurse (RN) Registered Nurse (Full Time ) Position Overview A Community Health Nurse works to promote the overall wellness of individuals and communities through education, advocacy and delivery of care. The CHN promotes healthy lifestyle choices by instituting health screenings, wellness projects and provides education on disease management and prevention. Developing relationships helps the CHN address health disparities within marginalized communities by focusing on the environmental, physical, and social determinants of health. The CHN also builds individual and community capacity through cultural mediation and by increasing the knowledge and self-sufficiency through a range of activities such as outreach, community education, and advocacy.
Location and Hours of Position This position will be working in Erie County, Crawford County and Venango County. Travel is required. General hours for the position are 8:30am-5:00pm.
Compensation Minimum starting wage is $27.00 per hour. PHN recognizes experience, education and site location when determining wage.
