Compensation

Minimum start is $28.00 per hour. PHN recognizes experience, education, and site location when determining wage.

Location and Hours of Position

PHN Titusville Community Health Center:120 S Martin St, Titusville PA 16354

The general office hours for this position are Monday to Friday 8:30 am - 5.00pm.

NO WEEKENDS

Traveling to PHN site in Oil City, Sandy Lake and Sheakleyville will be required.

Position Overview

The Registered Nurse (RN) Medicare Annual Wellness Visit Nurse is responsible for implementing Annual Wellness Visits and Chronic Care Management services at their assigned PHN sites. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or Welcome to Medicare) and Annual Wellness Visits (AWV) both initial and subsequent. Chronic Care Management (CCM) services consist of monthly activities designed to intensively manage chronic conditions of qualified patients and to coordinate all care that patient receives.

Qualifications

  • Registered Nurse (RN): Graduate of an approved school of nursing
  • Current unrestricted Pennsylvania Registered Nurse (RN) license
  • Current CPR Healthcare Certification
  • Ability to demonstrate competency in nursing skills, assessment and decision making
  • Knowledge, skill and ability to provide routine immunizations based on CDC and CDPH recommendations.
  • Knowledge, skill and ability to monitor vaccine storage and handling guidelines.
  • Ability to demonstrate self-discipline and work independently
  • Ability to work over a wide range of areas in a diverse community
  • Excellent communication skills both oral and written.
  • Excellent customer service skills.
  • Creative and analytical thinking
  • Ability to work under pressure calmly and efficiently, coping with variable workloads, intermittent delays and emotionally charged situations
