Back to Category List Available Positions > Job Description Annual Wellness Visit Nurse \| (RN) Registered Nurse (Full Time ) Location: Mercer County The Network The Primary Health Network is the largest Federally Qualified Healthcare Center (FQHC) in the state of Pennsylvania. We have been serving our communities for 36 years. We currently operate healthcare centers in 16 counties in the state of Pennsylvania and 1 in the state of Ohio. We offer primary care, behavioral health, specialty care (in select counties) and dental services across the state of Pennsylvania. Position Overview The Medicare Annual Wellness Visit Nurse is responsible for implementing Annual Wellness Visits and Chronic Care Management services at their assigned sites. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or "Welcome to Medicare") and Annual Wellness Visits (AWV) both initial and subsequent. Chronic Care Management (CCM) services consist of monthly activities designed to intensively manage chronic conditions of qualified patients and to coordinate all care that patient receives. Location and Hours of Position The general office hours for this position is 8:30 AM-5:00 PM. The position is located in Greenville, PA. Traveling to other Primary Health Network sites will be required -Sites include PHN Transfer, Sandy Lake, Sheakleyville. Education/Experience Requirements Licensed Practice Nurse (LPN) Graduate of an approved school of nursing Active Pennsylvania Nursing License Minimum of two years' experience in direct patient care (preferred) Case Management experience (preferred) OR Registered Nurse (RN) Graduate of an approved school of nursing Active Pennsylvania Nursing License Minimum of two years' experience in direct patient care (preferred) Case Management experience (preferred) Additional Job Qualifications/Skills Excellent interpersonal skills with patients Ability to effectively and collaboratively function within a medical office setting. Ability to abstract information, objectively analyze data, and have the organizational skills to prepare reports and gather supporting documentation. Excellent communications skills Proficiency with Electronic Medical Records (EMR) and Microsoft Suite.
