Centralized Care Coordinator  (Full Time )
Location: Mercer County

Position Overview

The Centralized Care Coordinator is accountable for scheduling patient referrals and testing with specialists and outside facilities, completing prior authorizations required for referrals, acting as a liaison between the patients and providers regarding referrals. This position will work collaboratively with patients, providers, clinical staff, outside facilities. They will also ensure timely scheduling and record exchanges, prompt communication between facilities and address any barriers that contributes to a gap of care.

Location and Hours of Position

The general office hours for this position are Monday-Friday, 8:30 AM-5:00 PM. The position is located in Sharon, PA.

Education/Experience Requirements

  • High School Diploma (or equivalent)
  • 2 years experience in a health care setting (required)
  • 2 years of clinical experience (preferred, not required)

  • Organized and Able to prioritize tasks
