Clinician - Outreach  (Regular Full-Time, Hourly)

Hours: 40 hours per week. Clinic hours are Monday, Tuesday, Thursday, Friday, 9:00 am - 5:00 pm; Wednesday 11:00 pm - 7:00 pm.

Pay Rate: $38.82 to $40.86 per hour

Education: Master's degree in a mental health-related field, such as Psychology, Social Work, or Counseling required

Licensure: Licensed or license-eligible with the California Board of Behavioral Sciences (BBS) is required

As a Clinician - Outreach, you will:

  • Conduct outreach at inpatient or START facilities to provide a warm handoff from inpatient to outpatient or appropriate level of services.
  • Develop and maintain a notification system with identified hospitals and START programs in order to provide efficient and effective outreach.
  • Conduct initial screenings and/or behavioral health assessments of new and/or returning clients; may be conducted via telehealth services or face-to-face.
  • Conduct risk assessments and provides crisis intervention as needed to ensure client safety.
  • Refer outreach clients to the appropriate level of service, including intensive case management provided at the program.
  • Perform outreach and follow-up with disengaged, high risk, or unconnected clients. Some work may be done via telehealth.
  • Work with clients to develop strengths-based service plan goals and objectives.
  • Services will be provided via clinic, telehealth, and mobile outreach.

Additional Qualifications and Skills

  • Minimum of two years of work experience with adults with serious mental illness
  • Must be able to work in a fast-paced and collaborative environment
  • A valid California driver’s license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business.
  • Able to pass criminal/background clearance

Heartland Wellness Recovery Center is one of our Adult Outpatient programs (AOP), which provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, peer support and older adult outreach. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders.

All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness.
