Clinician - SBCM (Strength-Based Case Management) - Mental Health Services for Adult Outpatient Program

Hours: 40 hours per week. Clinic hours are Monday, Tuesday, Thursday, Friday 9:00 am - 5:00 pm; Wednesday, 11:00 am to 7:00 pm.

Pay Rate: $33.65 to $36.06 per hour

Program Overview:

Located in south San Diego, CA, the Maria Sardinas Wellness & Recovery Center provides a variety of intervention services to individuals in crisis. We serve individuals with serious mental illness and co-occurring substance abuse disorders. Our services include initial psychosocial assessments, psychiatric and medication evaluations, ongoing medication support services, limited individual and group counseling, supported housing, care coordination, case management, and linking clients with additional community-based supports.

Typical Duties of the SBCM Clinician

  • Works closely with various providers in the region or system to facilitate access to program services for those identified as the target population to be served.  Primarily field-based position.  

  • Conducts initial and annual behavioral health assessment of new clients and/or returning clients. 

  • Performs collateral interviews as indicated.   

  • Provides individual and group psychosocial behavioral health services, case management/brokerage, and crisis intervention

    as needed. The activities and interventions use psychosocial rehabilitation (PSR) and recovery principles, as well as evidence-based practices that focus on goals specified in the client plan and includes case management, collateral contacts, and crisis intervention. 

  • Assists the client in developing client service plan goals, prioritizing areas of need, and steps needed to reach goals established, as well as identified areas of strength. Continually monitors progress towards goals and supports client’s efforts toward self-sufficiency.  

  • Bills and documents all services provided and maintains documentation in accordance with established policies and procedures in a timely manner. Documents all services provided and maintains documentation in adherence to County guidelines. 

  • Monitors and responds to Morning Reports and coordinates 48-hour discharge response from inpatient facilities/crisis residential facilities. 

  • Participates in weekly Service Planning meetings and staff meetings.   

  • Completes Outcome Measures per County requirements.  

  • Provides services within contract requirements, specifically adhering to the Strength Based Case Management

    model.  

Qualifications/Skills Requirement

  • Master’s degree (M.A. or M.S.) in a mental health related field and two years of work experience with adults with serious mental illness.
  • Licensed or license-eligible in California; must be kept current throughout employment
  • CPR/First Aid certification required and must be maintained throughout employment
  • Bilingual Spanish strongly preferred
  • Must be able to work in a fast-paced and collaborative environment
  • A valid California driver’s license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business.
  • Able to pass criminal/background clearance
