The Support Services Counselor will provide a variety of services to Veterans and their families. This position will provide counseling services to Veterans to develop and execute individual success plans to include career, housing, and financial goals. The person in this position will meet with potential participants and assess their eligibility by reviewing the required documentation for the program and determining eligibility. Ensure participants receive the assistance needed to promote housing stability, including but not limited to consulting with participants to conduct needs assessment, working with participants to create individualized housing stability plans including specific plans and specific goals, monitoring participants from the time of intake until they exit the program, and ensuring participants obtain needed supportive services, re-evaluate participant eligibility, per program requirements.

Complete a comprehensive assessment intake interview for potential participants both in person and via telephone to determine eligibility for program services.
Determine the participant's household needs and priorities. Gather and record information about the participant's health, income, eligibility for public benefits, employment skills, background, family relationships and supports, and living situation to identify and coordinate those services which will be most effective and acceptable to the participant in improving housing stability.
Refer ineligible persons to other appropriate service providers, maintaining documentation of the referral.
Interpret and explain information, such as eligibility requirements, application details, and program-specific information.
Develop a Housing Services Plan with each program participant who has qualified to receive program services.
Coordinate, prepare, and maintain required charting and documentation in a timely and thorough manner (including weekly HMIS input).
Coordinate with local resources to identify available and affordable units.
Promote the SSVF program to landlords and mediate any issues between participants and landlords.
Assist participants to coordinate their move-in, access furniture, and utility set-up as required by their lease agreements.
Periodically monitor the participants' progress on the Housing Services Plan and amend or extend the plan as needed (30, 60, 90 days) to ensure participants achieve their housing stabilization goals.
Provide follow-up appointments with all program participants monthly after their goals are met to assess whether they continue to maintain their permanent housing 90 days after stabilization.
Coordinate appropriate referrals and linkages to available community partnerships, and mainstream services such as Health Care Navigation and VA benefits.
Attend veteran events, SSVF program training, OIC’s training, and any other relevant training opportunities.
Maintain accurate and complete weekly case notes for each participant within the agency SSVF HMIS database. Perform routine backups to ensure program data is preserved.
Develop relationships with representatives in other agencies to support individuals attaining services such as additional mental healthcare, psychiatric medication, medical resources, financial assistance, legal advocacy, etc.
Work with other program team members to reach monthly goals for veteran enrollment into the program.
Participate in daily debrief sessions, weekly supervisory and/or staff meetings, training and development opportunities as determined appropriate by the program director.
Keep abreast of OIC of WA organization policies and procedures and keep the Program Director apprised of any concerns regarding upset participants.

A.A. Degree; BA degree in Social Services or related field
One to Two (1-2) years of experience in case management
Additional years of experience may be considered in lieu of education
Starting Salary: $23.96 per hour
