Alameda Health Consortium

Founded in 1974, the Alameda Health Consortium serves as the association for eight member community health centers that together serve more than 175,000 people in Alameda County. Services provided by the Alameda Health Consortium include: health policy and planning, advocacy, program development and implementation, training and technical assistance. We maintain a strong focus on promoting policies that increase access to health care, link individuals and families with health insurance, and reduce racial and ethnic health disparities.

Community Health Center Network

Community Health Center Network (CHCN) was formed by the community health centers in 1994 in order to more effectively participate in newly launched state managed care programs.  As a management services organization, CHCN now provides a wide range of administrative, clinical, financial, and information technology management services to our member health centers.  CHCN includes eight community health centers all committed to providing comprehensive quality health care in a manner respectful of community traditions and values.  Our health centers operate 30 primary care sites in Alameda, Contra Costa, Solano, and San Francisco counties.  CHCN continues to evolve with the changing health care environment to better support our health centers as they strive to improve the health of the communities they serve.
