Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care team.
This RN Case Manager Coordinator will coordinate and manage the nursing services of home care clients through development, implementation and evaluation of a plan of care, working in collaboration with the interdisciplinary team, specifically our agency's Visit Nurses, and ensure that services are completed at quality levels consistent with professional standards of practice.
In collaboration with the patient/caregiver, completes patient plan of care and submits admission paperwork.
Develop, schedule and communicate patient visit schedules to assigned Visit Nurses and ensure visits, plan of care are completed timely and within agency's quality standards.
Accurately and timely performs ongoing assessments of patients as evidenced by visit documentation, follows physician orders and plan of care revisions based on assessment findings.
Identify patient goals and develop a plan and implement interventions.
Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team members
Case conferencing with team members occurs regularly which includes creating PCW/HHA Care Plans, initiating and updating Plans in accordance with patient needs and Agency policy.
Plan of care is revised on a regular basis reflecting changes in patient's on going assessment and /or progress or lack of progress toward goals.
Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by Case Communication documentation.
Audit all clinical records upon recertification and discharge to evaluate content and compliance with State/Federal regulations and Agency policy and procedure.
Assure appropriate discharge planning occurs from the start of care by utilizing a multidisciplinary approach and appropriate documentation.
Coordinate with Prior Authorization Nurse the patients need for continuing services and providing accurate assessment data to support ongoing service.

As a Clinical Unit Coordinator you will assist the Hospice Manager and RN Supervisor in daily team operations to ensure efficient flow of communication and coordinatoion of services.
Professionally manage and problem solve incoming phone calls from patients, field staff, reference labs, physician offices, hospitals, nursing homes, home care coordinators and DME suppliers.
Create staff schedules, holiday schedules and weekend staffing and weekday master schedules.
Efficiently coordinate staffing coverage for all planned vacations, unexpected time off, PM and Weekend, and holiday coverage.
Track completion of admission, discharge paperwork and charting completed by the staff.
Maintain paper charts during patient stay and take apart upon discharge and forward to medical records.
Schedule and communicate daily volunteer tasks.
Effectively use internal reports, tracking processes and open communication with staff to assure team compliance with all policies/time line for all care related paperwork.

Gather and document data accurately to contribute to assessment of patient and to assure the integrity of the medical record. Report abnormalities to the RN.
Recognize areas where the patient's response deviates from the desired outcome and report to the RN.
Identify emergency situations and take appropriate action.
Reinforce aspects of patient teaching as delegated by the RN.

Provide appropriate companionship and maintains professional boundaries with patients and family members.
Perform errands and transportation needs for patient based on patients needs (clean vehicle; driving safety; being timely; appropriate money handling).
Perform meal preparation per patient standards including safety precautions for food preparation to include food safety (expired food), kitchen safety and meeting dietary needs/requirements.
Consistently complete office paperwork (mileage, PTO, availability forms) and Telephony according to Supervisor standards. Telephony must be completed at the start/end of every visit.
Communicate timely with team when patient is not at home for a scheduled visit, change in upcoming schedule and/or when need for assigned length of visit changes, adheres to assigned scheduled.

Complete patient plan of care and submit admission paperwork.
Perform ongoing assessments of patients as evidenced by visit documentation, follow physician orders and plan of care revisions based on assessment findings.

Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by Case Communication documentation.
Audit all clinical records upon recertification and discharge to evaluate content and compliance with State/Federal regulations and Agency policy and procedure.
Assure appropriate discharge planning occurs from the start of care by utilizing a multidisciplinary approach and appropriate documentation.
Coordinate with Prior Authorization Nurse the patients need for continuing services and providing accurate assessment data to support ongoing service.

Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care team.
As a Specialty Infusion RN you will coordinate and manage the services of infusion and intermittent home care clients through development, implementation and evaluation of a plan of care, working in collaboration with the interdisciplinary team and provide services at quality levels consistent with professional standards of practice.
In collaboration with the patient/caregiver, completes patient plan of care and submits admission paperwork within 72 hours.
Accurately and timely performs ongoing assessments of patients as evidenced by visit documentation, follows physician orders and plan of care revisions based on assessment findings.
Identify patient goals and develop a plan and implement interventions.
Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team member.
Case conferencing with team members occurs regularly which includes creating PCW/HHA Care Plans, initiating and updating Plans in accordance with patient needs and Agency policy.
Plan of care is revised on a regular basis reflecting changes in patient's on going assessment and /or progress or lack of progress toward goals.
Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by Case Communication documentation.
Audit all clinical records upon recertification and discharge to evaluate content and compliance with State/Federal regulations and Agency policy and procedure.

As a Speech Therapist, you will provide evaluation and therapy services in the homes of clients who require skilled intervention to restore function and prevent disability following disease, illness, injury or loss of a body part within Oak Creek, Racine, Kenosha service area. You will take an interdisciplinary team approach, under the direction of a physician.
Assess clients' physical, cognitive, behavioral, communication, emotional and social status; assess home safety, medication management and fall risk.
Assess clients' functional rehabilitation status to determine the current "level of functioning, self-care, self-responsibility, independence and quality of life" per JCAHO Rehabilitation Standards.
Submit accurate, complete and timely discipline assessments, OASIS data sets, physician orders and certifications / re-certification paperwork per Agency policy.
Select, apply and modify skilled interventions appropriate to the client's environment based on identified problems / needs, diagnoses, prognosis, anticipated goals, desired outcomes and client treatment choices.
Establish a plan of care with interventions that reflect current, valid research findings supporting appropriateness to meet clients' needs.
