Responsible for networking, educating and relationship-building with current and potential referral sources.

Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care team.
ThisRN Case Manager Coordinatorwill coordinate and manage the nursing services of home care clients through development, implementation and evaluation of a plan of care, working in collaboration with the interdisciplinary team, specifically our agency's Visit Nurses, and ensure that services are completed at quality levels consistent with professional standards of practice.
In collaboration with the patient/caregiver, completes patient plan of care and submits admission paperwork.
Develop, schedule and communicate patient visit schedules to assigned Visit Nurses and ensure visits, plan of care are completed timely and within agency's quality standards.
Accurately and timely performs ongoing assessments of patients as evidenced by visit documentation, follows physician orders and plan of care revisions based on assessment findings.
Identify patient goals and develop a plan and implement interventions.
Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team members
Case conferencing with team members occurs regularly which includes creating PCW/HHA Care Plans, initiating and updating Plans in accordance with patient needs and Agency policy.
Plan of care is revised on a regular basis reflecting changes in patient's on going assessment and /or progress or lack of progress toward goals.
Inform physician, clinical manager and interdisciplinary team members of changes in patient status as evidenced by Case Communication documentation.
Audit all clinical records upon recertification and discharge to evaluate content and compliance with State/Federal regulations and Agency policy and procedure.
Assure appropriate discharge planning occurs from the start of care by utilizing a multidisciplinary approach and appropriate documentation.
Coordinate with Prior Authorization Nurse the patients need for continuing services and providing accurate assessment data to support ongoing service.

Works collaboratively with Agency leadership to implement new service opportunities as identified
Demonstrates working knowledge of all aspects of home care reimbursement including Medicare, Medicaid, Managed Care and Commercial Insurance through managing the process of patient payers and authorization.

Explain to patient the care being provided and involves patient in cares.
Consistently complete mobile care upon arrival and departure from patients' home per Agency policy. Mobile Care includes tasks completed in accordance with the care plan. Complete mileage logs timely and accurately.
Work cooperatively with patients, care providers and family members, as well as with appropriate Agency staff by listening and responding to patient needs, engaging in appropriate communication, reporting pertinent patient information to Case Manager in a timely manner
Regularly complete home maintenance per plan of care i.e., house cleaning, washing dishes, laundry, meal preparation, and transportation, etc.

As aPrivate Duty Home Health Aideyou willprovide direct assistance with activities of personal care, maintenance of patient health, Agency facilitation of treatment for patients, and contribution to a health environment for patients. Consistently complete mobile care upon arrival and departure from patients' home per Agency policy. Mobile Care includes tasks completed in accordance with the care plan. Complete mileage logs timely and accurately.
Work cooperatively with patients, care providers and family members, as well as with appropriate Agency staff by listening and responding to patient needs, engaging in appropriate communication, reporting pertinent patient information to Case Manager in a timely manner

Serves as a resource/point of contact for referral sources by conducting one-on-one visits to build and maintain relationships that result in census growth.
Provides weekly reports on contacts generated (i.e., physician, hospital Case Manager, key office staff, etc).
Sets up programs yearly to enhance referral relationships.
Maintains a current knowledge base of all the services provided by Horizon Home Care & Hospice.
Assists the Director of Marketing and Sales with: The annual marketing/sales plan and participating in health fairs, job fairs, exhibitions, etc.
Supports and demonstrates a customer service commitment to internal and external customers through adherence to Horizon Home Care & Hospice customer service standards and processes.
Participatessalesmeeting and program/team meetings when invited.
Ensures conflict resolution with referral sources, Agency staff and patients.
Meets with patient's and families to consult regarding services provided by Horizon.
Meets goals as established through the Agencys planning/budgeting process.

Our Homemaker Companions take the time to truly understand our clients needs; so whether its deep cleaning bathrooms and kitchens, organizing closets and cupboards, sharing family recipes while preparing meals, enjoying the summer breeze while gardening, or simply providing safe transportation to and from appointments, our Homemaker Companions will be there.
As aHomemaker Companionyou will:
Perform necessary Homemaker Companion services in patient's home by conducting housekeeping per the directive (bathrooms, kitchen, laundry, vacuuming/washing floors, dusting, etc.).
Provide appropriate companionship and maintains professional boundaries with patients and family members.
Perform errands and transportation needs for patient based on patients needs (clean vehicle; driving safety; being timely; appropriate money handling).
Perform meal preparation per patient standards including safety precautions for food preparation to include food safety (expired food), kitchen safety and meeting dietary needs/requirements.
Consistently complete office paperwork (mileage, PTO, availability forms) and Telephony according to Supervisor standards. Telephony must be completed at the start/end of every visit.
Communicate timely with team when patient is not at home for a scheduled visit, change in upcoming schedule and/or when need for assigned length of visit changes, adheres to assigned scheduled.

As part of an interdisciplinary Hospice team, theHospice Social Workeris responsible for the delivery of caries social work services to Hospice patients and their families. The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, directs casework services, bereavement services, and community education outreach and referral. Additionally you will:
Make home visits and conduct case conferences with other staff members.
Perform comprehensive psychosocial, financial and bereavement assessments, assist in the development and implementation of goal directed family care plans and follow up with clinical intervention.
Provide assistance with long-range planning.
Respond to referrals by internal and external sources in a timely fashion.
Assume the active role of advocate for the patient/family; facilitate open communication between patient, family unit, physician and interdisciplinary staff on a regular basis and in a timely fashion.
Provide short-term crisis intervention and individual/family counseling.
Identify potentially complicated grief process in family or significant others and develop a plan of care which reflects incorporation of current, valid research findings to establish appropriate interventions to meet client needs.

Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care team.
As a Specialty Infusion RN you will coordinate and manage the services of infusion and intermittent home care clients through development, implementation and evaluation of a plan of care, working in collaboration with the interdisciplinary team and provide services at quality levels consistent with professional standards of practice.
In collaboration with the patient/caregiver, completes patient plan of care and submits admission paperwork within 72 hours.

Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team member.

As the Volunteer Services Specialist, your purpose is to ensure that the hungry and the homebound are cared for in our community. This meaningful position will allow you the opportunity to connect with community members and volunteers, feel accomplished with varied day-to-day responsibilities, and collaborate alongside a team of enthusiastic individuals.
Responsible for the oversight of three community-focused programs, which include: Meals on Wheels, Final Hours, and Lifeline. The Volunteer Services Specialist works closely with the volunteers who make these programs operable by providing basic communication and retention and complete required clerical maintenance and impeccable customer service. Some duties of this position include:
Meals on Wheels (MOW) program:
Welcome new clients to the MOW program
Oversight and retention of volunteers
Schedule weekly routes and assign volunteers to deliver meals
Maintain meal pick-up area
Ensure service agreement forms are completed, process prior authorization, and finalize billing information via HomeCare HomeBase
Final Hours program:
Prompt communication with hospice team and hospice families upon receipt of request
Oversight and retention of volunteers
Schedule volunteers Monday - Friday
Submits orders into HomeCare HomeBase
Lifeline program
Welcome new clients to Lifeline
Contribute to the promotion of Lifeline
Schedule Lifeline delivery, installation, and pick-up; covers shifts as necessary
