In this important role you will efficiently coordinate the daily functions of our clinical team and assures effective staffing of our home care clients. You will professionally represent Horizon to external customers and act as a resource for field staff. Sometimes you're a traffic director, a cheerleader, a drill sergeant, or a computer data entry whiz. Outstanding organization, customer service, and computer skills are essential!

  • Perform meal preparation per patient standards including safety precautions for food preparation to include food safety (expired food), kitchen safety and meeting dietary needs/requirements.
  • Consistently complete office paperwork (mileage, PTO, availability forms) and Telephony according to Supervisor standards. Telephony must be completed at the start/end of every visit.  
  • Communicate timely with team when patient is not at home for a scheduled visit, change in upcoming schedule and/or when need for assigned length of visit changes, adheres to assigned scheduled.

Medical Social Worker - Hospice  (Full Time 1st shift)

Our Hospice team is looking for a compassionate, personable Medical Social Worker to join our Hospice Team within Racine and Kenosha Counties in Southeastern Wisconsin.

As part of an interdisciplinary Hospice team, the social worker is responsible for the delivery of caries social work services to Hospice patients and their families.  The purpose of this position is to provide initial psychosocial evaluations, ongoing psychosocial counseling, directs casework services, bereavement services, and community education outreach and referral.  

  • Make home visits and conduct case conferences with other staff members.
  • Perform comprehensive psychosocial, financial and bereavement assessments, assist in the development and implementation of goal directed family care plans and follow up with clinical intervention.
  • Provide assistance with long-range planning.
  • Respond to referrals by internal and external sources in a timely fashion.
  • Assume the active role of advocate for the patient/family; facilitate open communication between patient, family unit, physician and interdisciplinary staff on a regular basis and in a timely fashion.
  • Provide short-term crisis intervention and individual/family counseling.

Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care Team.

As a PM Visit Nurse you will be responsible for ensuring the delivery of quality home care services as well as the coordination of services that reflect the philosophy and standards of home care health and nursing care of assigned patients.  Provides services at quality levels consistent with professional standards of practice.

  • Assess patient conditions and family / caregiver's conditions accurately and completely during each visit (conditions may include physiological, psychosocial and / or spiritual).
  • Identify and provide for patient and family's educational needs.
  • Provide patient care per physician orders and Agency policy.
  • Complete all required patient and Agency paperwork in an accurate, thorough, legible and timely fashion to reflect a clear picture of care provided.
  • Transcribe, implement and evaluate new physician orders accurately.
  • Field staff schedules, payroll and daily logs are accurate, legible and timely.
  • Work cooperatively with patients, care providers, family members and Agency staff.  
  • Communicate patient's issues and concerns to the Case Manager, appropriate health care professional, and / or Clinical Manager in a timely and accurate manner.

RN - Residential (Kathy's Hospice)  (Part Time PMs)
Location: West Bend (Kathy Hospice)(53095)

In Horizon’s Hospice Inpatient Units, you will play an important role in supporting and educating patients and their families as they embark on the end-of-life journey. You’ll draw on all parts of your experience: science and other knowledge from school, skills you’ve developed throughout your career, and most importantly: compassion and empathy.

You also will experience a culture of family with your peers and the company itself, which is rare to find.  We find that flexibility, supporting our team members, assisting in their professional growth and providing a positive environment help our employees find a long time home for their skills.  

We are seeking experienced nurses to join our Kathy's Hospice Inpatient team.   

  • Perform referral evaluations / admission assessments of patients and families seeking Hospice services.  
  • Performs physical, psychosocial and spiritual assessments utilizing non-verbal cues and probes or modifies questions to obtain more comprehensive assessments
  • Communicates patient’s and family’s expectations of care, goals and comprehensive learning needs to all members of the health care team via appropriate documentation too
  • All documentation is clear, legible, concise, accurate, timely, and meets department standards
  • Provide Case Management functions, as part of the IDG, to an assigned case load. 
  • Provide services at quality levels consistent with professional standards of  practice.  Approach patients and families with care, concern and compassion.
