ACT Case Manager  (Full Time)
Location: Woonsocket, RI

Our Assertive Community Treatment Team is looking for a Case Manager who has experience working in the substance abuse and mental health field. You will be an integral part of our multidisciplinary team to help people avoid crisis situations and to keep crises from turning into unnecessary hospitalizations. ACT is designed to help those individuals who are struggling with serious and persistent mental illness and co-occurring substance abuse disorders that may need more than the traditional outpatient mental health care and psychiatric rehabilitation more intensive services. As the Case Manager will provide comprehensive community support to those individuals living in their homes. Your role will be to help stabilize individuals by assessing and evaluating all clients on an ongoing basis, provide support services and/or crisis intervention as needed, develop, design and implement treatment goals/plan with the client to promote the individual's recovery. In addition, collaborate with community resources while promoting recovery, improving their mental health and rehabilitation. This is a full time position with benefits.

Education/Licensing Requirements: BA/BSW in a human services related field required. Knowledge of substance abuse and mental health is a plus. ACT Substance Use Specialist  (Full Time)

Within this role you will be responsible for managing a multi-disciplinary team. We are looking for candidates that have 2-3 years solid community based experience working with adults with severe and persistent mental illness, demonstrate leadership capabilities, excellent oral and written communication skills and are comfortable working in a fast-paced environment serving a population with complex bio-psychosocial needs. Successful candidates must also be able to work within a systems context collaborating with internal Community Care Alliance programs as well as other community providers. You must also have training and experience with providing supervision, case management, crisis intervention, advocacy, counseling, consultation, substance abuse treatment, health education and other supportive services.

Acute Services Counselor  (Full Time)
Location: Providence,RI

The Acute Services Counselor is an integral part of our Acute Stabilization Unit (ASU) in Providence.  This crisis service and hospital diversion or step-down program provides a relaxing, protected and supervised non-hospital setting.  You will work in a multi-disciplinary team in the assessment and initial treatment planning phase for program clients. Providing clinical case management services to identified clients experiencing psychiatric and/or substance abuse problems. The right candidate will have experience working with and treating co-occurring/substance abuse populations. The Counselor will coordinate discharge planning, serve as a liaison to community mental health centers and transition identified clients into appropriate after-care services. Must have a minimum of six months of community mental health experience.  Position is 11am-7pm.

Behavioral Health Worker- ASU  (Full Time)
Location: Providence,RI

If you are interested in gaining experience working in the mental health and substance abuse field we have the perfect position for you! Our Acute Stabilization Unit in Providence is looking for a candidate to work as a Behavioral Health Worker/CNA.  This is a 3 day work week, 36 hour position. The unit works from a trauma informed care practice model. Our program has qualified staff that are trained to realize the widespread impact of trauma, recognize signs/symptoms, respond with knowledge, and seek to resist re-traumatization. You would be responsible for ensuring patient safety, demonstrating professional behavior, and keeping established rules/protocols while maintaining patient dignity. As the Behavioral Health Worker/ CNA you will also be responsible for assisting in treatment groups and activities on the unit.

You would be exchanging pertinent client information to team members and working under the supervision of the Registered Nurse and Program Manager. Must have excellent written and communication skills and the ability to multi-task while working in a fast-paced environment. Being safety focused, detail oriented, and the ability to respond to emergencies or crises is an important aspect of this position.

 

Education/Licensing Requirements: High School Diploma or GED required.  Associate's degree in Human Services or attending school in a Human Services discipline. Certified Nursing Assistants with the right experience in Human Services and current CNA students or college students are encouraged to apply!

Searching for a Per Diem Security Officer/ Treatment Assistant to staff our 24/7 Behavioral Health Link – Crisis Services Program. Community Care Alliance and Horizon Health Partner's, is looking to hire an individual with experience in providing security services for this program. While security and program safety will be the primary responsibility of this position, the expectation is for the employee to assist in other program duties as assigned. Good communication and verbal de-escalation skills are a must.

The Behavioral Health Link is a 24/7 behavioral health crisis and triage center which provides Rhode Islander's with immediate assessment and referral services. The program is staffed by a multi-disciplinary team which consists of RN's, Psychiatry, master's level counselors, case managers, peer specialists and phone screeners. The program also operates a 24/7 suicide hotline as well. The mobile crisis unit is a program transit van which is available to crisis center staff to provide community home visits when needed.

Education and Licensing Requirements: Associate degree preferred, but will interview HS graduates with relevant experience.

The Care Coordinator is a navigator with the family, working with families in their home and in the community, collaborating with natural supports and other service providers to assist the family in identifying their needs and goals.  Must be able to address crisis issues & help to stabilize families, as well as safety-planning when needed, as well as ongoing monitoring for child safety and risk factors.  The FCCP is strength based and family driven, with the Care Coordinator conducting a strengths-based, culturally competent family needs assessment.  The ongoing work is to help the family to meet their goals while increasing their protective capacity with their children, so the children can remain with their parents.

The Care Coordinator will also meet the requirements of documentation for client records and conduct other assessment tools with the family.  The Care Coordinator will participate in Wraparound and other required trainings and become wrap certified within one year of employment.

Intake Specialist- Community Support Program  (Full Time)
Location: Woonsocket, RI

Our Community Support Program needs a compassionate individual to fill the position of Intake Specialist.  This role is responsible for the lead in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health coordination and prevention activities.  CSP provides case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community.  You will assist in orienting new clients and their families to CSP and health home services.  Looking for an individual who can demonstrate competency in clinical/technical or administrative skills, able to work as a member of a multi-disciplinary team, and can perform in a fast paced environment.  This position does require transporting clients and performing home visits. IOP/PHP Counselor  (Full Time)

We are looking to obtain a 3rd shift Registered Nurse for 36 hour full-time, Crisis Stabilization Unit in Providence, RI.  We are seeking an empathetic, caring, and energetic RN to deliver the best care possible to adults in various stages of recovery from mental illness, substance use disorders, and chronic medical conditions.  RN will assess and monitor the psychiatric and medical needs of clients, provide telephone crisis intervention to clients experiencing behavioral health issues, and conduct assessments.  Responsible for assisting clients to access medications needed for stabilization and educate on the effects of medication.  Collaborate with CSU team, providers and families to provide comprehensive services.  Evaluation of clients for hospitalization in crisis and this includes implementing a plan for safety as well as facilitating voluntary and involuntary hospitalization when necessary.  There is also opportunity to teach and facilitate daily groups.

The Crisis Stabilization Unit at Providence works from trauma informed care practice model. This model works to realize the widespread impact of trauma, recognize signs/symptoms of trauma, respond to trauma with knowledge, and seeks to resist re-traumatization.

Relief Residential Treatment Workers  (Per Diem)
Location: Various - Rhode Island

Seeking to enhance our pool of per diem workers at various locations throughout Northern RI. We are able to offer FLEXIBLE hours during 1st, 2nd, 3rd and weekend shifts (this includes mother's hours). First and second shifts are $13 and $14 per hour, depending on location. Third shift, overnight awake positions, are $15 an hour, the Pascoag group is $17 per hour.

Programs are looking for Residential Per Diem Workers to assist in providing case management services to adult residents in our Residential & Supported Housing programs. These positions assess and evaluate clients on an ongoing basis, providing support services and/or crisis intervention as needed. In this position you will also assist clients in medication monitoring and education, conduct security checks on each room on an hourly basis, and other administrative duties as needed.

Education/Licensing Requirement: HS diploma or equivalent required with experience in areas of mental health/substance abuse. Associate’s degree in Human Services or prior work experience preferred.  Position requires you to obtain CPR and First Aid if you do not have a current certification. Jellison House, our men’s residential program in Johnston, RI is looking for a Residential Treatment Worker to work on a per diem basis. Serenity Peer Recovery Specialist- Paid Intern  (Internship)

Transportation Specialist - NRIVC  (Part Time)

The Visitation Center serves families whose children are in the care of the Department of Children, Youth and Families due to substance use or mental health concerns. Families are able to have supervised visits in a home-like setting while receiving support in their recovery. The Transportation Specialist provides transportation to and from supervised visits with biological families for children in out-of-home placements. Will communicate with foster parents and overall facilitation of scheduling visits with all parties involved. This position is also responsible for ensuring the safety of passengers at all times, including documentation for state, program, and agency standards and the implementation of individualized transportation plans. You would be exchanging pertinent client information to team members and working under the supervision of the Registered Nurse and the Program Manager. Candidate must have excellent written and communication skills, ability to handle crises, and ability to multi-task while working in a fast paced environment. Being safety focused, having attention to detail, and being able to respond to emergencies or crisis is an important aspect of this position.

Education/Licensing Requirements: High School Diploma or GED required.  Associate's degree in Human Services or attending school in a Human Services discipline.
