ACT Vocational Specialist  (Full Time)

 The ACT Team is conducting a search for a full time Vocational Specialist to begin immediately. ACT is a program to help individuals avoid crisis situations and to keep crises from turning into unnecessary hospitalizations. The multidisciplinary team is designed to provide comprehensive community support to those individuals living in the community. The team works with individuals who are struggling with serious and persistent mental illness and co-occurring substance abuse disorders that may need more than the traditional outpatient mental health care and psychiatric rehabilitation services more intensive support.

As a Specialist you will focus on vocational assessment, job development, coaching, and engagement. Your role will also include creating treatment plans with clients, attending team meetings, and collaborating with community providers regarding clients’ needs. This position is looking for an individual who can work independently, but also as a team player! Prior experience in employment services is preferred. Also looking for a candidate who has previous experience working in the mental health or substance abuse field.

Community Care Alliance is seeking managerial candidates for its Assertive Community Treatment Team serving severely mentally ill adults, many with co occurring substance issues. Within this role you will be responsible for managing a multi-disciplinary team. We are looking for candidates that have 2-3 years solid community based experience working with adults with severe and persistent mental illness, demonstrate leadership capabilities, excellent oral and written communication skills and are comfortable working in a fast-paced environment serving a population with complex bio-psychosocial needs. Successful candidates must also be able to work within a systems context collaborating with internal Community Care Alliance programs as well as other community providers. You must also have training and experience with providing supervision, case management, crisis intervention, advocacy, counseling, consultation, substance abuse treatment, health education and other supportive services.

Acute Services Counselor - Providence  (Full Time)
Location: Providence,RI

The Acute Stabilization Counselor is an integral part of our Acute Stabilization Unit (ASU) in Providence.  This crisis service and hospital diversion or step-down program provides a relaxing, protected and supervised non-hospital setting.  You will work in a multi-disciplinary team in the assessment and initial treatment-planning phase for program clients. Providing clinical case management and group treatment services to identified clients experiencing psychiatric and/or substance abuse problems.  Holistic group approaches utilized such as meditative exercise, smoothie/nutritional groups, music, art, etc. The right candidate will have experience working with and treating co-occurring/substance abuse populations. The Counselor will coordinate discharge-planning, serve as a liaison to community mental health centers and transition identified clients into appropriate after-care services. Must have a minimum of six months of community mental health experience.

Acute Services/ASU LPN  (Full Time)
Location: Providence,RI

POSITION SUMMARY:

The LPN will provide direct care services to clients admitted to the ASU. Services include medication administration, coordination with local pharmacies in accessing medication for clients, implementing and administering detoxification protocols, ordering supplies in coordination with RN’s; and conducting nursing assessments on all new admissions.   Community Care Alliance is seeking to add a motivated, task oriented, enthusiastic case manager to their Integrated Health Home Team!  This position is interwoven into the Adult General Outpatient Department thus there will be no requirement to work evening or weekend hours.  Our mission is to provide individuals with wraparound supports and services in order to improve and stabilize functioning so clients can achieve independence!

 


Typical job duties include:

 

Coordinating with a multidisciplinary team consisting of case managers, nurses, substance use specialists, clinicians and psychiatrists to enhance client care.

 

Providing transportation to individuals, as necessary, such as to medical visits, court appearances, to pick up prescriptions etc.

 

Providing assessment and referrals to various levels of care based on presenting concerns for mental health and/or substance use

Our LIHEAP Department is searching for a Community Coach to conduct assessments for clients of the LIHEAP Program who are seeking assistance.  This will include long term monitoring of clients, providing energy conservation, financial education, and workforce development as needed.  Attend community events of various types in order to extend services through other social service agencies and community partners.  Maintaining relationships with collaborative partners and cultivating new ones. Responsible for collecting and analyzing data. Working with clients to educate individuals on becoming economically self-sufficient.  Offering supportive counseling and advocacy to clients and their families, providing resume building, how to conduct a job search, and best practice interviewing workshops.  We are looking for a strong advocate who is interested in supporting client’s efforts to have their voice heard and to help them obtain the services they need to help them become self-sufficient.

Awake Overnight Residential Treatment Worker-Wilson House  (Full Time)
Location: Pawtucket, RI

An Awake Overnight Residential Treatment Worker is needed for our men's recovery program in Pawtucket.  At Wilson House men have the opportunity to focus completely on recovery from chemical dependency in a home-like atmosphere. This program is highly structured and emphasizes relapse prevention.  Sober and drug-free lifestyles are supported and we offer services that can become a part of your support system after discharge as well.  As a Treatment Worker you will assist in the implementation of case management services, programs and activities. Behavior Interventionist  (Full Time)

The unit works from a trauma informed care practice model. Our program has qualified staff that are trained to realize the widespread impact of trauma, recognize signs/symptoms, respond with knowledge, and seek to resist re-traumatization. You would be responsible for ensuring patient safety, demonstrating professional behavior, and keeping established rules/protocols while maintaining patient dignity. As the Behavioral Health Worker/ CNA you will also be responsible for assisting in treatment groups and activities on the unit.

You would be exchanging pertinent client information to team members and working under the supervision of the Registered Nurse and Program Manager. Must have excellent written and communication skills and the ability to multi-task while working in a fast-paced environment. Being safety focused, detail oriented, and the ability to respond to emergencies or crises is an important aspect of this position.

 

Education/Licensing Requirements: High School Diploma or GED required.  Associate's degree in Human Services or attending school in a Human Services discipline.  BH Link is looking for a highly motivated and outgoing individual to join our Mobile Crisis Assessment Stabilization Team (MCAST) as a Per Diem Mobile Clinician.  BH Link is a 24/7 behavioral health crisis and triage center which provides individuals who are 18 and older with an array of services including crisis assessments, referrals to additional resources, and coordination of care with outside providers.

The MCAST model is centered on community outreach and assessment facilitated by a mobile team which includes a Masters Level Clinician and an Emergency Medical Technician (EMT).

 

Education and Licensing Requirements:

Applicant must have a master’s degree in the human service field with at least 1 year of experience working within a behavioral health, inpatient, or acute care setting.  Knowledge of trauma informed care is a plus.

We are currently seeking an LCDP or license eligible chemical dependency professional to serve as the team’s substance use case manager on our family wellness team titled, “Promoting the Integration of Primary and Behavioral Healthcare (PIPBHC).” This case manager will use a strengths-based trauma informed approach to help support clients in their recovery from substance use disorders and co-occurring mental health issues. In addition, this case manager will also work with family members to provide psychoeducation regarding substance use disorders, and improve overall health and wellness of the entire family unit through extensive case management support and advocacy. This individual must be well organized, a self-starter, and have excellent communication skills.

 

Typical Job Duties Include:

· Collaboration with participant’s family members and other members of the team in the development of a person centered recovery focused treatment plan

· Model and support health and wellness activities and healthy coping skills for PIPBHC families

· Provide case management support and advocacy to participants and their families to eliminate any barriers to participant’s recovery; Interventions including advocacy, resource and benefit linkage, symptom management, coaching and skills building, support and rehab interventions

· Asses clients on an ongoing basis, provide support services and crisis intervention as needed

· Work collaboratively with the other members of the team and with other systems, including the regional FQHC which may be involved with the PIPBHC families.

· Use a strengths based trauma informed approach to help support clients in their recovery from substance use disorders and co-occurring mental health issues.

· Provide care coordination between providers in various settings to improve client and family participation and engagement

· Provide individual and family treatment for children, adults and family members who meet PIPBHC criteria

Duties include: outreach to local agencies in East Bay Area: (Bristol, Barrington, Warren, and East Providence police and EMS departments, as well as agencies in the Northern RI area: (Woonsocket, Cumberland, Lincoln, North Smithfield and Burrillville police and EMS departments) to encourage/educate agencies to refer to our community mental health center 24/7 services. Complete data entry, collection, and reports. Attendance at meetings with Regional Prevention Task Force and Rhode Island Department of Behavioral Healthcare, Developmental Disabilities, Hospitals (BHDDH) as scheduled. Additional responsibilities include providing Face to Face emergency assessments in the community on an as needed basis. This is a unique opportunity to not only market this program but to be able to participate in providing direct care in the community. Family Support Partner - Youth Specific FCCP  (Full Time)

POSITION SUMMARY:

The FCCP is a family preservation program. Services are provided by community agencies and funded by DCYF. We provide service to any youth/family with child under the age of 18. Services are intended to address risk for involvement with DCYF or law enforcement. The program can assist with navigating services to address developmental, behavioral, and social problems, as well as families who are at risk for abuse or neglect.

The Youth Specific Family Support Partner acts as a peer mentor to youth and a support to families. As a member of the Wraparound team, this youth support partner will help them to navigate systems, access resources, and connect to natural supports. Candidates must have lived experience with youth services or equivalent youth related work.

This position is Youth Specific Support. For youth referred for Wayward or Youth Diversion, weekly contact with the youth is required. This support partner will mentor youth, spending time with youth building rapport, building skills or completing tasks to address needs and goals identified by the FCCP plan. Youth specific FSP would also coordinate with the Care Coordinator and other team members, especially parents, to address barriers and communication issues.

 

The Youth Specific Family Support Partner works under and receives regular direction (weekly supervision) from a program Clinical Supervisor or designee, according to the Agency’s mission and philosophy, Program, and Division goals. FSPs may work in conjunction with multiple team members on assigned cases and must have excellent written and verbal communication skills.

Family Visiting Referral/Admin Assistant- Bilingual  (Full Time)

POSITION SUMMARY:

  The purpose of this position is to provide telephone and, on occasion, face to face referral response for the First Connections program and clerical duties for other early childhood Family Visiting programs (i.e., Early Intervention, Healthy Families America) The referral process consists of gathering all necessary information via web, fax, phone and mail delivery to determine program interest and eligibility, as well as scheduling the first appointment for direct services. The Referral Specialist enters referral information and disposition in the FC Referral log and web-based RIDOH portals. This staff contacts the referred family to discuss the program intake, schedule first appointments and subsequent appointments if needed. This staff also works closely with direct service staffers to maintain schedules appropriately and coordinate with other Family Visiting programs when multi program referrals are made. Clerical duties include filing, mailings, copying, organizing billing documentation for data entry and other administration tasks as required.

EDUCATION AND TRAINING REQUIRED:

 High school diploma, as well as experience in telephone and face to face interviewing. Working knowledge of program and intake processes. Must have an ability to interact in a professional and courteous manner in all contacts with clients, community agencies and staff. Bi-lingual capacity in Spanish/English is required.

WORKING CONDITIONS:

 · Knowledge of modern office procedures and methods including telephone/text communications, office systems, and record keeping.

· Knowledge of proper business communication, including style and format of letters, memoranda, minutes, and reports.

The Rapid Re-Housing program is seeking a case manager who works closely with enrolled clients to support individualized goals that help establish and maintain stable housing.  The case manager and clients identify appropriate resources to achieve goals and remove barriers in order to improve self-sufficiency and independence in assessing resources.  This program is designed to work with people who have been impacted by economic hardship and have become homeless. 

Responsibilities Include:

Master’s Level Clinician, 3 Day Work Week - BH Link  (Full Time)

Outreach Peer Recovery Specialist  (Full Time and Part Time)

The Serenity Center/Safe Haven is a thriving Recovery Community Center, providing recovery supports to individuals in the Northern RI area.  We are looking for individuals with lived experience with substance use, co-occurring mental illness, and/or homelessness who have been in recovery for at least a year.  Our members are across the spectrum of recovery, from precontemplation to maintenance.  We work with them at our two drop-in centers, through group and individual services.  This includes time in the community meeting people where they are at using a harm reduction approach.  If you are not already certified, we will support your participation in the Certification for Peer Recovery Specialist (CPRS) training.  Not only will you earn direct service hours you will receive both individual and group peer supervisions to assist you in developing as a professional.  You will work one-on-one with members to provide peer support, learn how to facilitate groups and participate in planning and delivering sober social events.  This is an immersive experience offering the opportunity to learn the day-to-day operations of a recovery center.

 We are looking for a Registered Nurse to work full-time on our Acute Stabilization Unit located in Providence, RI.  We are seeking an empathetic, caring, and energetic RN to deliver the best care possible to adults in various stages of recovery from mental illness, substance use disorders, and chronic medical conditions.  As part of our multidisciplinary team, RNs assess and monitor the psychiatric and medical needs of clients, provide crisis intervention to clients experiencing behavioral health issues, and conduct assessments as part of the admission process.  RNs are also Responsible for conducting detoxification protocols and administrating psychotropic medication.  All new nursing employees receive 20-30 hours of education on mental illness, psychotropic medication, trauma informed care, group treatment and crisis intervention as part of the orientation process. RNs are also eligible to attain certification as a Qualified Mental Health Professional at the conclusion of the 3-month orientation training process. New graduates are Welcomed!

Relief Residential Treatment Workers  (Per Diem)
Location: Various - Rhode Island

Seeking to enhance our pool of per diem workers at various locations throughout Northern RI. We are able to offer FLEXIBLE hours during 1st, 2nd, 3rd and weekend shifts (this includes mother's hours). First and second shifts are $13 and $14 per hour, depending on location. Third shift, overnight awake positions, are $15 an hour, the Pascoag group is $17 per hour.

Programs are looking for Residential Per Diem Workers to assist in providing case management services to adult residents in our Residential & Supported Housing programs. These positions assess and evaluate clients on an ongoing basis, providing support services and/or crisis intervention as needed. In this position you will also assist clients in medication monitoring and education, conduct security checks on each room on an hourly basis, and other administrative duties as needed.

Education/Licensing Requirement: HS diploma or equivalent required with experience in areas of mental health/substance abuse. Associate’s degree in Human Services or prior work experience preferred.  Position requires you to obtain CPR and First Aid if you do not have a current certification. Candidates must have an approved driving record with the ability to transport clients.

Residential Case Manager- Singleton House  (Full Time)
Location: Pascoag, RI

Our residential program in Pascoag is in need of an Awake Overnight Treatment Worker to provide case management services. This position will assess and evaluate clients on an ongoing basis, providing support services and crisis intervention as needed. As the Overnight Awake position candidates must have an approved driving record with the ability to transport clients. Responsibilities include; assisting clients in medication monitoring and education, filing client information into records, conducting hourly checks on rooms, maintaining required documentation and daily program communication. Assist in providing case management services in accordance with the resident's individual treatment plan, goals, philosophy and standards defined by supervisor and CCA. 

 

Education/License Requirements: Associate’s degree in Human Services related field required, or a combination of education or relevant work experience that is equivalent. BA or attending college in Human Services related field with two years of experience in areas of mental health/substance abuse preferred. Position requires you to obtain CPR and First Aid certification.
