POSITION SUMMARY:

Exciting opportunity to join a dynamic behavioral health team! If you are interested in gaining experience working in the mental health and substance abuse field, we have the perfect position for you. Our Acute Stabilization Unit in Providence is looking for a candidate to work as a mental health treatment assistant. No mental health experience needed as this is an entry level position. We have 3 and 4-day work weeks available, 36 hours full-time.  This position works directly with individuals experiencing a mental health or substance abuse crisis. Treatment assistants are responsible for ensuring patient safety, demonstrating professional behavior, and keeping established rules/protocols while maintaining patient dignity. As a treatment assistant you will also be responsible for assisting in treatment groups and activities on the unit as well. Treatment assistants also assist clients in activities of daily living ( showering, laundry, meals, etc.) and conduct client safety checks/rounds to observe and record behaviors as part of their daily duties.

Must have excellent communication skills and the ability to multi-task while working in a fast-paced environment. Being safety focused, detail oriented,and the ability to respond to emergencies or crises is an important aspect of this position.

POSITION SUMMARY:

Ensure and monitor general safety and structure of the shelter as they pertain to shelter rules and policies. Do routine rounds of the facility and monitor any safety concerns. Monitor any situations between residents at the shelter and be able to practice de-escalation and conflict resolution amongst residents. Be able to conduct appropriate outreach to any safety resources and emergency services. Additionally, ensure general safety of the residents by ensuring the building structure is properly cared for and maintained.

 

- complete circuits of the property every 30 minutes for each shift

- Observe and record any potential safety concerns regarding the building, procedures, and/or individuals (residents and guests) at the shelter.

- Exercise the ability to de-escalate conflict between clients and perform any crisis intervention that may cause a concern for safety or one or multiple individuals.

The 988 Crisis Call Center also coordinates with local mobile crisis response teams, dispatching available mobile crisis clinicians to the community in order to meet people where they are. The services provided emphasize client-centered care in the least restrictive possible settings.

- Working on site at the 988 Call Center in East Providence.

- Answering calls, chats, and texts alongside Crisis Counselors (through the 988 Lifeline and the 414-LINK line). This will include:

This position is based in a drop-in center for youth ages 14-24 from diverse backgrounds and experiences. We are in need of a dynamic individual that is passionate about working with this age group and hard to serve individuals. The Academic coordinator is our bridge between The Harbour Youth Center, the local middle/high schools and community partners. This individual would be the person on the ground to engage youth, educate the schools on the resources available to youth and create programming that excites our students. Currently seeking an experienced and motivated Assistant Team Leader for our ACT Team. This position shares in a leadership role managing a multi-disciplinary team that includes case managers, specialists, nurses and clinicians. Co-Leads in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health management and prevention activities.  Working in collaboration with the team leader, ACT staff and other staff personnel to ensure the clinical and administrative duties of the team promote effective treatment to clients. 

 

Additional duties include providing counseling and case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. 

 

EDUCATION AND TRAINING REQUIRED: 

 

· Master’s degree in a human service-related field required. 

· Licensed or working towards licensure (i.e., LCSW/LICSW, LMHC, LCDP) 

· Two years of experience in a community mental health center preferred. 

· Ability to participate as a member of a multi-disciplinary team 

· Approved driving record with ability to provide transportation and home visits.

ACT Peer Specialist  (Full Time)
Location: Woonsocket, RI

 A Peer Specialist for our Assertive Community Treatment Team(ACT) is needed to provide a bridge between providers and adults with serious mental illness. Clinician - Healthy Transitions  (Full Time)

The team will take a progressive and collaborative approach to its work with young adults with the goal to develop innovative practices to meet their needs in a holistic manner.

The successful candidate will be dynamic in their approach, have previous experience and knowledge about the continuum of healthcare and social services available to children, young adults, and their families, and be familiar with the needs and treatment of persons with serious mental illness. You will work to protect the safety, health and welfare of residents in the program while promoting their independence, dignity and individuality.  Must communicate effectively to assist clients in performing Activities of Daily Living, work simultaneously on several tasks at one time, and be able to communicate with outside providers.

Education and Licensing Requirements:  Current certification to practice as a medication aide in Rhode Island and current certification as a Certified Nursing Assistant required. Join a committed team of professionals!  FCCP works with families who have a child with behavior issues or diagnosis, where the family is not sure how to get help.  Other families are eligible if they are at-risk for involvement with the Department.  All families are in need of assistance in navigating services or community resources that may be available to help their family and utilizing family and natural supports to overcome barriers.  The goal of FCCP is to strengthen families in becoming more self-sufficient.

The Care Coordinator is a navigator with the family, working with families in their home and in the community, collaborating with natural supports and other service providers to assist the family in identifying their needs and goals.  Must be able to address crisis issues & help to stabilize families, as well as safety-planning when needed, as well as ongoing monitoring for child safety and risk factors.  The FCCP is strength based and family driven, with the Care Coordinator conducting a strengths-based, culturally competent family needs assessment.  The ongoing work is to help the family to meet their goals while increasing their protective capacity with their children, so the children can remain with their parents.

The Care Coordinator will also meet the requirements of documentation for client records and conduct other assessment tools with the family.  The Care Coordinator will participate in Wraparound and other required trainings and become wrap certified within one year of employment.

POSITION SUMMARY:

This position will provide case management and direct support to individuals residing at the North Smithfield Shelter. The case manager will provide services to assist residents in stabilizing their situation and exercising a successful transition to independent and self-sufficient living. This individual will provide resources and advocacy to individuals while working on a plan for housing. The Shelter Case Manager will provide shelter-based services establishing and maintaining a service plan that guides their involvement in the program. The case manager will be expected to connect the family to needed services and coordinate those services and referrals guided by the case management plan.

 

 

 

EDUCATION AND TRAINING REQUIRED:

High School Diploma/GED and relevant experience. Ability to work independently and within a team. Interest in ongoing training, supervision, and professional development. Bi-lingual skills welcome.

 

 

WORKING CONDITIONS:

· Provide services in shelter, in the community, and within the agency.

· Maintain a valid driver’s license and own reliable transportation.

· Work in team environment

 

 

SPECIFIC FUNCTIONS:

· Deliver case management services to assigned clients within the shelter and any related programs as assigned by supervisory staff

· Complete all required documentation in a timely manner

· Carry a case load of 12-15 individuals as needed

· Provide referrals/ care coordination

· Assist in room inspections, floor meetings, conflict resolution

· Keep appointments with clients in the office

· Attend team meetings, agency-wide meetings and community meetings as assigned

· Conduct case conferences related to client progress

· Assist in all Performance Quality Improvement efforts related to shelter programs

· Other duties as assigned by supervisors

Tanguay Awake Overnight Treatment Worker  (Full Time)

An Awake Overnight Residential Treatment Worker needed for our Tanguay Housing Program in Woonsocket. The Awake Overnight Worker will assist in providing case management services to residents in accordance with the resident's individual treatment plan, goals, philosophy and standards defined by the agency. This position will assess and evaluate clients on an ongoing basis providing support services and/or crisis intervention as needed. Your 3rd shift duties include assisting clients in medication monitoring and education, conducting security room checks on an hourly basis and filing client information into records.

Education/Licensing  Requirements: Associate degree or working towards degree in Human Services with 2 years’ experience preferred. High School diploma or equivalent required if there is a number of years of experience in the mental health/substance abuse field.
