Family Services Care Coordinator - BBL213 (Full Time) - Woonsocket, RI

We are seeking an enthusiastic person to fill the position of Family Service Care Coordinator on our Wraparound team which is part of the Northern Region Family Care Community Partnership (FCCP). The Family Service Care Coordinator will work with a variety of families with children and youth who are at risk of abuse and neglect, who have serious emotional disturbance and/or who have juvenile correction involvement. The FSCC will serve as a wraparound facilitator and will assist families with obtaining the necessary supports to ameliorate risk factors. Skill in collaborating with school personnel is essential for this position as many clients will be referred by the Positive Educational Partnership. This position requires flexibility, a willingness to work in a variety of settings and the ability to be team member.

Spanish speaking individuals are encouraged to apply. Wraparound training will be provided.

 

 

Education Requirements: Bachelor’s Degree in Social Work or similar field is preferred, a combination of education and experience will be considered. Preference will be given to individuals with personal experience navigating the system of care.

Residential Treatment Worker (Per Diem-LB32, PT-LB44/LB46,FT ) - Johnston, RI

These positions will assist in providing services to adults at Jellison House, a sixteen bed recovery residence for men and women who have substance abuse and mental health issues. Assess and evaluate clients on an ongoing basis providing support services and/or crisis intervention as needed. Group and case management experience required.  

Education Requirements: Bachelor's degree or working towards degree with 2 years experience preferred. High School diploma or equivalent required with experience in areas of substance abuse and mental health. Candidates must have an approved driving record with the ability to transport residents.

License/Certification Requirements: Preference giving to candidates who have or are in pursuit of a CIT or LCDP certification. Food Certification a plus. Position requires you to obtain CPR and First Aid certificate (Sponsored by NRICS).

CSP Casemanager - MD291 & MD292 (2 Full Time) - Woonsocket, RI

Our progressive community support outreach program seeks two case managers to provide counseling and case management services to psychiatrically disabled adults and families in order to maintain and improve their level of functioning in the community. Duties include:

·  Provide case management services to clients in their home (residence), at the Center, in the community and other institutions, (such as ACI, hospitals, substance abuse treatment settings.)

·  Assess and evaluate all clients on an ongoing basis, providing support services and/or crisis intervention as needed

·  Develop, design and implement treatment goals/plan with client

Spanish speaking abilities and knowledge of substance abuse a plus. Ability to work across multidisciplinary teams, driver's license, approved driver's record, and reliable transportation required. One year experience in community mental health preferred.

Education Requirements: AA in a human services field required; BA/BSW in a human services related field preferred.

License/Certification Requirements: Valid driver's license. Position requires you to work towards CCSP after 6 months of employment (sponsored by NRICS)
